Five steps. One care team.

The same care team, from the first screening to the refill.

Step 1 and 2

Screened as part of Chronic Conditions care. A physician reviews the screening and decides whether GLP-1 is clinically appropriate.

Step 3 and 4

If it is, the physician sets the treatment plan. A health coach works with the member alongside it.

Step 5

Labs and refills are handled by the care team, so the member does not start over.

Common questions.

Who decides whether GLP-1 treatment is right?
A U.S.-licensed physician, after reviewing the member's screening. GLP-1 treatment is offered only when it is clinically appropriate.
What is the AHA CKM framework?
The American Heart Association's Cardiovascular-Kidney-Metabolic framework for integrated chronic condition care. Our program is built on it.
What does our organization see?
Aggregate, de-identified screening yield by risk level and program engagement. Individual results stay between the member and their care team.
